Biometric Screening Event
Well Wisconsin Biometric Screening Event
When: Monday, April 18th
Time: Appointments available between 8:00 a.m.-1:00 p.m.
Where: University Union, Phoenix Rooms
Who can Participate: Member employees and spouses/domestic partners enrolled in State Group Health Insurance. Note: As of January 1, 2016 adult children are no longer eligible to participate or receive the Well Wisconsin Incentive.
How to Participate: Pre-Registration is required!

Deadline to Register: Thursday, April 14 (or until all appointments are filled)
In 2016, all state health insurance plans are offering $150/employee and spouses/domestic partners for completing the Well Wisconsin program. There are two steps to completing the program.
- Participate in a biometric screening
Get up-to-date testing results on your height, weight, and Body Mass Index (BMI), cholesterol level, glucose measurement, and blood pressure. At the on-site screening you will be asked to provide proof of insurance (health insurance card). You will then go through a series of staffed stations for the testing, which generally takes less than 20 minutes. At the end of the screening you will be given your results.
- Complete your Health Risk Survey online provided by your health plan
Use your biometric screening results to complete the survey. Visit http://wellwisconsin.wi.gov and click on your insurance plan in the column to the left for a direct link to the online Health Survey. This quick, easy survey will ask a range of questions including your daily stress level, family medical history, and typical meal plan. The Health Survey is designed to help you gauge intangible well-being factors to pinpoint possible health risks.
Once the biometric screening and health survey are completed, the $150 incentive will be processed and issued by your health insurance provider. Health plan issued rewards are considered a fringe benefit of employment and must be treated as taxable income by the primary subscriber’s employer. Your health plan is required to report payment information for the subscriber and enrolled family member’s to the subscriber’s employer. Individual health information will never be shared with the employer.
Even if you are unable to attend the on-site Biometric Screening on April 18th, you can complete the biometric screening portion with your physician (the necessary paper forms for physician’s completion are available under each individual health plan on the Well Wisconsin website).

We are looking forward to enjoying fresh vegetables and herbs during their 15 week harvest season from mid-June through mid-September. CSA Wellness Lunch ‘n Learn
Would you like to enjoy fresh-picked locally grown veggies and herbs each week? Please come to this Lunch ‘n Learn to find out more about CSAs! Community Supported Agriculture (CSA) is a partnership between a farm and a local community of supporters that provides a direct link between the production and consumption of food. CSA members cover the farm’s operating cost by purchasing a share of the harvest. This provides members with a healthy supply of locally grown, seasonally fresh produce throughout the growing season.
Sleepy Hollow Farm & CSA is a small, farmer owned farm in Kewaunee County. They specialize in growing a variety of veggies using sustainable farming practices, and no chemicals, herbicides or pesticides touch your food. Presenter Suzi Sevcik has a lifetime of farm experience. From growing up on her parent’s dairy farm to now working the fields with her own farm. Using techniques that are kind to the earth to grow veggies, she takes the approach of never ending improvement. Trialing different varieties to see what grows well in their micro-climate, to constantly find better ways to grow veggies and manage weeds and pests. The mission always remains the same – To produce nourishing food that respects that land, and feeds our community and to exemplify a sustainable model for small-farm viability.
