Information Services provides computerized analysis of course evaluations. All course evaluations must be accompanied by the Course Evaluation Cover Sheet.
The above form should be included with completed course evaluation forms, which are provided by your department’s support person. Departments use different types of course evaluation forms, CCQ’s (Course Comment Questionnaires), SQANAL (short questionnaire analysis), and forms designed for specific needs.
A student volunteer must read the instructions to the class, collect completed evaluation forms, SIGN THE FORM, and deliver the envelope to Media Services (IS 1010). An evening drop box, between IS 1010 and IS 1012, is provided for “after hours” drop offs. Please remind the student delivering the completed evaluations to sign the STUDENT EVALUATION OF TEACHING COVER SHEET.
Only Course Evaluations with signed cover sheets will be certified.
Course Evaluations should be filled out at the end of each semester, approximately two weeks before final exams. Media Services is responsible for the processing of evaluations and for notifying the department chair/support person when data is ready for pickup.
Because of the volume of requests, we require at least fifteen business days (following final exams) to process the forms. For special requests (Unit Chairperson), a 2-day notice is appreciated.
